Accompanying original pension application were following affidavits filed 12 May 1879--
28 Apr 1879-Affidavit by Daniel Greenleaf, physician, of Bloomfield, Davis Co, Iowa, attests he knew Mr COOK prior to his enlistment in service and was known to be in good health.

28 Apr 1879-Affidavit by Private Albert G Clyman, member Co D, 3rd Iowa Cavalry Volunteers, of Davis Co, Iowa, attests he served under Capt Norman COOK; that Capt COOK enlisted and served as Capt of Company D from 1861 until discharged 14 Jun 1863; that in late May or early June 1862 portion of Company D were detailed by Col Bussey to escort a commissary train from Batesville, Arkansas to Rolla, Missouri and that Capt COOK was thrown from his horse during that escort resulting in the rupture of his navel.

29 Apr 1879-Affidavit by George P Clark, of Van Buren Co, Iowa, private, member Co D, 3rd Iowa Cav Vol, attests Capt COOK served from Aug 1861 until 14 Jun 1863 as Capt of Co D; that portion of Co D escorted commissary train from Batesville, Ark to Rolla, Missouri; Capt COOK was thrown from horse during escort resulting in rupture of navel in late May or early June 1862.

30 Apr 1879-Affidavit by William C Niblack (Niblock), 2nd Lietenant, Co D, 3rd Iowa Cav Vol, of Schutler Co, Missouri, attests Capt COOK lead escort from Batesville to Rolla, though Liet. Niblack did not accompany escort. Also states that Capt COOK received rupture to navel when thrown from horse during escort. After injury while Capt COOK remained in service, he was not on duty, but referred to occasionally. Capt COOK frequently complained that he was unfit for duty. Mr Niblack knew Capt COOK from abt 1850.

2 May 1879-Affidavit by W L Orr of Ottumwa, Wappelo Co, Iowa, assistant surgeon 3rd Iowa Cav. Indicates Capt Cook found afflicted with umbilical hernia, upon examination.

8 May 1879-Affidavit by Norman COOK attesting to his condition; resident of Red Oak, Montgomery Co, Iowa, 52 years old [in 1879] , enlisted 24 Aug 1861 & mustered in as Captain of Co D, 3rd Reg Iowa Cavalry Volunteers, commanded by Col Cyrus Bussey; honorably discharged at Helena, Arkansas 14 Jun 1863 by surgeon certificate of disability-surgeon's description[in 1863]: 35 yrs, 5'10"; complexion dark, hair black, eyes hazel. After discharge resided in Davis and Montgomery counties, occupation as merchant much of the time. Indicated that he is now permanently disabled and requested that he be placed onto the invalid pension.
